Colonoscopy preparations
I’m supposed to get a colonoscopy (oh joy:) ) which means 24 hours on
clear liquids. When I called to schedule it they started telling me
about all the soft drinks and fruit juices on the approved list. Right.
So does anyone have suggestions for 24 hours on clear liquids.
(T2, max metformin, mas (I think) glyburide, actos, 10 or 15 units of
Lantus at bedtime (to avoid the dawn phenomenom)
Thanks for any ideas.
ADA Conference Items
Our local pumper’s club had its annual ADA scientific meeting update
by one of the local endocrinologist last night. A couple of the
highlights. This is based on my inadequate notes and faulty memory of
what is always a session punctuated with many questions that tends to
interrupt narrative clarity.
For the type 2s – Progress continues apace on the analogs to GLIP-I
(Glucagon Like Peptide I). GLIP-I increases insulin output,
regenerates damaged beta cells, slows glucose absorption into the
blood, and causes weight loss. Diabetics show loss of production of
GLIP-I. The down side of GLIP-I is that it is short lived. A shot
starts working in 10 minutes and it is gone in an hour. Everybody and
their grandmother are looking for long acting analogs. Farthest in
the pipeline is Exenatide (the famous lizard spit peptide) for which
phase 3 trial data was released at the meeting. The results include
reductions in A1c of 1 to 1.5 points, continued weight loss through 52
weeks, no additional hypoglycemia, and beta cell regeneration.
Treatment requires 2 subcutaneous shots/day. Roll out is expected in
June 2005. Wide use in the type 2 community is expected. Several
other analogs are starting trials including one that binds to albumin
but remains active and one with a few amino acids switched that
doesn’t break down. Others are working on inhibiting the breakdown
mechanism with some success.
Type 1 & 2 – Many (most?, all?) complications appear to ultimately
result from oxidation problems in the mitochondria. New very powerful
antioxidants are in the works. Vitamin C and E are useless in this
regard. In fact, they are strongly contraindicated in diabetics
taking statins. Prospective trials have shown C and E to counter the
secondary beneficial actions of statins.
Type 1 & 2 – DIAD Trial (Detection of Ischemia in Asymptomatic
Patients With Diabetes) 75-80% of diabetics die of adverse
cardiovascular events. 50% of those have no chest pain to trigger
emergency call. For 1/3, death is the result of their first
cardiovascular event. DIAD is an attempt to find those at risk who
are missed because they do not show symptoms. The diagnostic test is
a stress heart scan. 20% of those scanned have a clear cardio
vascular problem. 1/3 of those have a major problem which requires
treatment, e.g. stent placement. There will be 4 more years of follow
up to see if the detection and treatment result in improvements in
mortality. The presenter is screening all his 50+ year old diabetics
every 2 years. If you are clear in the stress heart scan, you have a
very low chance of a problem in the following 2 years.

Australian researchers studied over 20,000 patients who
went to the hospital with a heart attack or other
coronary emergency. Nearly a quarter of the patients who
didn’t experience chest pain were misdiagnosed and under
treated. And these patients were 3 times more likely to
die in the hospital compared to patients who did feel
chest pain.
8% of the patients studied had acute coronary problems
including heart attacks, unstable angina and clots that
blocked blood flow to the heart without experiencing
chest pain. Their most common symptoms were: shortness of
breath, profuse sweating, nausea, vomiting and fainting.
Patients without chest pain were older, were more likely
to be women and were more likely to have a history of
high blood pressure, diabetes or heart failure. They were
less likely to have a history of smoking, high
cholesterol or angioplasty or bypass.
23% of painless patients were given incorrect initial
diagnosis compared to just 2% of those with chest pain.
13% of patients without chest pain died in the hospital
compared to 4% of those with chest pain.
Researchers used data from an international registry of
over 20,000 heart patients for their study. The study was
led by researchers from Concord Hospital, Sydney,
Australia. It is published in the journal "Chest".

Latest research suggests that pain-free hearts attacks
are more deadly than the painful ones.
What’s more the research, published in Tuesday’s edition
of medical journal Chest, found that one in 12 heart
attack victims suffers a silent one increasing the
possibilty of misdiagnosis or improper treatment.
The study found that patients with atypical symptoms were
10 times more likely to be misdiagnosed and three times
more likely to die than those who suffer chest pain.
The study was conducted by a team at the St Michael’s
Hospital in Toronto.
The victims of silent heart attacks do not experience the
typical crushing pain in the chest. They have more
moderate symptoms such as shortness of breath, excessive
sweating, fainting, nausea and vomitting and nagging pain
in the shoulder or jaw.
The study also found that women over 65 tend to more
prone to silent attacks than men in the same age group.
The risk increases when combined with chronic conditions
like diabetes.
Lead researcer cardiologist David Brieger of Sydney’s
Concord Hospital said the findings should serve as an
alarm bell for doctors have to be alert and open-minded
while diagnosing heart attacks.
"Often, when a patient arrives at the hospital without
chest pain, it is only after blood test results come back
or other diagnoses are excluded that the physician
reassesses the situation and realises it is an acute
cardiac event after all," he said.
"We hope our findings will remind physicians that these
events do occur in the absence of chest pain and will
prompt them to make the diagnosis and institute the
appropriate treatment more rapidly."
A heart attack occurs when a coronary artery, which
supplies oxygen and nutrients to the heart muscle,
becomes blocked by a buildup of fatty plaque. To conduct
the study, researchers analysed data from 20,881 patients
in 14 countries who were hospitalised with a variety of
health conditions between July, 1999 and June, 2002.
Of the 1,763 patients who had heart attacks without
experiencing chest pain, 13 per cent died in hospital,
compared to 4.3 per cent of those with chest pain.
Almost 24 per cent of patients without chest pain were
misdiagnosed when they arrived at the hospital, compared
to only 2.4 per cent who had typical symptoms.

Effect of 1 tbsp of honey on my BG
This morning I wanted to see what effect honey has on my BG. I’m Type 2.
Hasn’t eaten at least 10 hours before.
My BG was 129 at start, before I eat one table spoon of Honey, 16 gr of
carb, according the labels info.
15 min later 178 BG
another
15 min later 166 BG
another
20 min later 126 BG back to the original reading.
So, looks like in 50 min burned up the 16gr carb of honey.
Did any one do similar test and how does it compare to mine?
Recent "Men’s Health" magazine had an article, that 2-3 tbspn of Honey per
day is very healthy for diabetics.
Contains very important minerals.

Bay area weekly warns of increased childhood onset of Type 2 Diabetes
I saw this in the Newsstand today and thought it was a very good
article. I only found a couple of errors, or misguided statements. All
in all a fine article in a free weekly.

which glucose monitor system is less painful
I recently was diagnosed with Type II Diabetes and I would like to
start monitoring my blood sugar level. I would like to know which
glucose monitor system is the least painful (I don’t like to prick my
finger, especially multiple time a day). Thanks.
HbA1C calculation
I use an excel spreadsheet to record my BG readings. Over time I have added some other features
to calculate the
1. Average readings for each waking hour,
2. Average readings for each day, and
3. A guesstimate of the HbA1C reading by taking a 90 day average of the daily averages.
It struck me during a rest period between sessions at the gym that the HbA1C reading may not be a
simple average of the daily averages but may be a weighted average with current BG readings
contributing more than readings taken up to 13 weeks ago.
I tried googling for info on this but came up with nothing that helped.
We could do the calculation in 2 ways if there is a linear drop off in the amount that each daily BG
reading contributes to the final figure.
1. The readings for the current day could count 100% towards the calculated figure and each
preceding day contributing n x 1.1% less where n is the number of days prior to the current day.
That would involve 90 parts to the formula.
2. A simpler method would count the weekly average readings for the current week 100% towards the
calculated figure with those in each previous week contributing n x 7.7% less where n is 1 for the
previous week, 2 for 2 weeks ago etc etc out to 12 weeks. The simpler method would still need 13
parts to the formula.
If the drop off is non linear does anyone have the formula?

Maximum antiinflammatory activity of phytic acid (PA) was seen at an oral dose
of 150 mg/kg in the carrageenan induced rat paw edema model. Although PA showed
ability to prevent denaturation of proteins, it showed less antiinflammatory
activity than ibuprofen. Ability of PA to bring down thermal denaturation of
proteins might be a contributing factor in the mechanism of action against
inflammation. PA, at all the doses tested, showed significant protection from
ulcers induced by ibuprofen, ethanol and cold stress, with a maximum activity
at 150 mg/kg. There was a significant increase in gastric tissue
malondialdehyde levels in ethanol treated rats but these levels decreased
following PA pretreatment. Moreover, pretreatment with PA significantly
inhibited various effects of ethanol on gastric mucosa, such as, reduction in
the concentration of nonprotein sulfhydryl groups, necrosis, erosions,
congestion and hemorrhage. These results suggested that gastro-protective
effect of PA could be mediated by its antioxidant activity and cytoprotection
of gastric mucosa.
